Family Health West honored former employee Andy Surface by dedicating a bike rack in her memory on July 31. Surface was a physical therapy assistant who worked at the FHW inpatient rehab department for eight years before passing away in September 2025.

Family Health West said in a news release that Surface loved making any adventure or event with friends and family special. She could bring people together and make them smile and laugh.

“The FHW Bike Rack is a wonderful tribute to Andy and the love and respect the team had for her,” said Taylor Surface, Andy’s younger brother. “She rode her bike to work often and would have loved this safe, covered place for her ride. Having this beautiful, custom bike rack will remind everyone who knew her what they shared.”

The new bike rack is in the courtyard outside the hospital, allowing patients and employees access to a safe and meaningful place to store their bikes. The bike rack was designed to represent several different elements of Andy’s life. The sun as the centerpiece serves as a reminder of her love for the beauty of nature. The kayak paddles, the hiker, the Monument spire, and the skis were Andy’s outdoor passions. The panda is both one of Andy’s nicknames and the name of her last dog.

“I am thankful for the team at FHW and the beautiful tribute they created for my sister,” Taylor Surface said. “I’m glad she found Fruita and the caring people at the hospital she got to finish her career with.”
