Intermountain plans to build primary-care clinic in Fruita

Fruita clinic designed to deliver full spectrum of primary and walk-in care

Intermountain Health plans to build a primary-care clinic in Fruita, according to a “General Project Report for IMH MGWC Fruita Primary Care Clinic” that it submitted to the City of Fruita in 2025.

The report’s Major Site Plan summarized Intermountain Health’s plans “to construct a 15,500-square-foot medical office building located at 502/524 Kokopelli Boulevard in Fruita, Colorado.”

The adjoining lots are vacant, and the Mesa County Assessor’s website shows Sisters of Charity of Leavenworth Health System Inc. (SCL Health) purchased each lot in February 2023, paying $550,000 for the 1.47 acres at 502 Kokopelli Blvd. and $1,500,000 for the 1.43 acres at 524 Kokopelli Blvd. Each sale was recorded by the Mesa County Clerk and Recorder on Feb. 9, 2023.

SCL Health merged with Intermountain Healthcare in 2022 and formed Intermountain Health, which operates Intermountain Health St. Mary’s Regional Hospital in Grand Junction.

The site plan shows Intermountain Health proposes to construct a medical-group clinic, and “the initial phase of development will establish a comprehensive 8-provider clinic, with site and building planning accommodating a future Phase 2 expansion.”

The plan also says, “Fruita Clinic will deliver a full spectrum of primary and walk-in care, complemented by rotating specialty services in obstetrics, general cardiology, and orthopedics. The facility program includes four primary-care providers, two dedicated walk-in providers, and two rotating specialists, supported by on-site imaging, including X-ray and ultrasound, and dedicated treatment rooms.”

The plan lists primary access to the facility will be from Kokopelli Boulevard, and a second access will be from Mesa Avenue. The project calls for 68 parking spaces.

The project report notes a water-quality facility is proposed at the southwest corner of the site, and it will discharge to the stormwater infrastructure in Kokopelli Boulevard.

A small City of Fruita Planning and Development sign on the 502/524 Kokopelli Blvd. property provides public notice about a requested pedestrian-easement vacation. The sign lists a Fruita Planning Commission hearing date of May 12, 2026, and a Fruita City Council hearing date of June 16, 2026.

On March 27, architect Altus Architectural Studios submitted a permit application for a commercial building at 502 Kokopelli Blvd. under the project name “Intermountain Health Fruita Primary Care Clinic.”

The Business Times requested comment from Intermountain Health about the proposed Fruita clinic, but Intermountain Health Media Relations said the person who could comment about the project was out of the office during the days prior to The Business Times’ publishing deadline.

The General Project Report said, “The applicant anticipates starting construction in the Summer of 2025 and finishing the project in fall of 2026.” Intermountain Health Media Relations acknowledged those dates are outdated, and details about the building may have changed.
